BACKGROUND: Deficits in neuromuscular control are common after anterior cruciate ligament (ACL) reconstruction and may be associated with further knee injury. The knee valgus angle during a single-leg squat (SLS) is one measure of neuromuscular performance. PURPOSE: To determine whether the knee valgus angle during SLS changes between 6 and 12 months after ACL reconstruction and to assess how the operative knee valgus angle compares with that of the contralateral side. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A cohort of 100 patients with uninjured contralateral knees were assessed at 6 and 12 months after primary hamstring autograft ACL reconstruction. Participants performed the SLS on each leg, and the knee valgus angle was measured via frame-by-frame video analysis at 30° of flexion and at each patient's maximum knee flexion angle. RESULTS: For the operative limb at 30° of flexion, a small but statistically significant reduction was noted in the valgus angle between 6 and 12 months (5.46° vs 4.44°; P = .002; effect size = 0.24). At 6 months, a slightly higher valgus angle was seen in the operative limb compared with the nonoperative limb (5.46° vs 4.29°; P = .008; effect size = 0.27). At maximum flexion, no difference was seen between limbs in the valgus angle at either 6 or 12 months, and no change was seen in the operative limb between 6 and 12 months. At 6 months and 30° of knee flexion, 13 patients had a valgus angle greater than 10°. This group also had a higher mean valgus angle in the contralateral limb compared with the contralateral limb in the other 87 patients (8.5° vs 3.65°; P < .001). CONCLUSION: During a controlled SLS, the knee valgus angle remained essentially constant, and minimal limb asymmetries were present over the 6- to 12-month postoperative period, a time when athletes typically increase their activity levels. Whether changes or asymmetries will be seen with more dynamically challenging tasks remains to be determined. When present, high valgus angles were commonly bilateral.

PURPOSE: It was the aim of this study to analyze the clinical and radiographic outcome after early arthroscopic repair regardless of the age of patients. METHODS: Patients with massive traumatic cuff tear and clinical pseudoparesis for forward elevation treated by subsequent early arthroscopic repair from 2011 until 2014 were included in this retrospective study. Exclusion criteria were Goutallier grade ≥3 fatty infiltration and prior shoulder problems or surgery. Magnetic resonance imaging (MRI), radiographs, and functional assessments were performed preoperatively and at follow-up. RESULTS: A total of 21 patients (male/female 15/6; age range: 30-83) were included. Preoperative MRI showed complete 2 tendon tears in 7 patients, 3 tendon tears in 13 patients and all tendons ruptured in one patient. All patients had full passive range of motion and the mean active elevation was 35.7° (range: 0°-60°). Nine patients also had a pseudoparesis for external rotation (mean: 10°, range: -30° to 40°). The mean delay until surgery was 33 days (range: 13-60). At follow-up (mean: 39 months, range: 24-60) all patients showed reversal of pseudoparesis, mean elevation of 165.2° (range: 110°-180°) and mean external rotation of 49.3° (range: -20° to 80°). The mean Constant score was 82 points (range: 56-95), and the mean subjective shoulder value was 93% (range: 50-100). The overall retear rate was 20% (n = 4). Fatty infiltration increased at least 1 grade in patients who had a retear and in 56% of patients (n = 9) without retear. Age was not a predictor for retear. CONCLUSIONS: This study shows that early arthroscopic repair of traumatic massive RCT with pseudoparesis may lead to successful results regardless of patients' age. A complete restoration of the function can be expected even in patients with retear. The retear rate is low and the increase of fatty infiltration minimal. LEVEL OF EVIDENCE: Level IV, case series.

Problems of the proximal tibiofibular joint (pTFj) after high tibial osteotomy (HTO) are rare. With this case report, we strive to highlight the importance of investigating the pTFj in patients with unexplained knee pain after HTO. A 44 year old male patient presented with diffuse pain on his left knee 3 years after medial opening wedge HTO due to medial compartment overloading in a varus knee. Patient described persistent anterior tibial and lateral knee pain. 2 years after HTO, patient underwent implant removal but the knee pain persisted. As the reason for the persistent pain was not identified, further radiological evaluation was done. Single photon emission computerized tomography/computerized tomography (SPECT/CT) revealed that there was no increased uptake within the tibiofemoral joint, indicating a biologically well performed correction of the varus deformity. However, markedly increased tracer uptake was found at the pTFj. On the inherent axial CT scans, it was seen that the proximal screws were too long and placed within the pTFj. Along with this a severe osteoarthritis of the pTFj was identified. The cause of the patient's pain was then confirmed by a CT guided infiltration of local anesthetic. An arthrodesis of the pTFj was performed and at 12 months followup after the arthrodesis the patient was pain free. This case highlights how important it is to evaluate the pTFj in patients with unexplained pain after HTO. SPECT/CT was helpful in identifying the patient's problem in this challenging case.

AIM: We investigated whether genetic polymorphisms may contribute to the interpatient variability of cisplatin-induced nephrotoxicity. PATIENTS & METHODS: Polymorphisms in the candidate genes GSTM1, GSTT1, OCT1, OCT2, LARP2, ERCC1, XRCC1 and EPO were analyzed for associations with nephrotoxicity in 79 cancer patients receiving cisplatin-containing chemotherapy. RESULTS: Higher cisplatin dose was associated with strongly decreased estimated glomerular filtration rates (eGFR) (r(2) = 0.205). Two highly genetically linked polymorphisms in the ERCC1 gene, 8092C>A and Asn118Asn, were significantly associated with change in eGFR, accounting for an additional 13% of interindividual variability. Homozygous carriers of the 8092A allele in ERCC1 showed no reduction in eGFR, compared with the 11.5% mean eGFR decrease in C allele carriers (p = 0.004). Homozygous carriers of the C allele of Asn118Asn showed no reduction in eGFR, compared with the 12.8% mean eGFR decrease seen in T allele carriers (p = 0.047). Polymorphisms in the other candidate genes were not associated with cisplatin-induced nephrotoxicity. CONCLUSION: Genetic polymorphisms in ERCC1 may be valuable predictors of cisplatin-induced nephrotoxicity.
